Specialist Delivery Practitioner –Family Hubs Navigator

Unique Ways is a local Parent Carer led charity supporting families with disabled children, helping them to lead ordinary lives. The aims of our charity are 2-fold:-
• Provide support directly to Parent Carers in Calderdale (both general and specialist support services) which helps them build their resilience and enable them to become more empowered.
• Ensure that Parent Carers have effective voice and influence in the design, development and delivery of local services through our voice and influence project; Family Voice Calderdale (FVC), which is the designated Parent Carer Forum for Calderdale and part of the National Network of Parent Carer Forums (NNPCF).
Unique Ways is working in partnership with Family Hubs Calderdale to focus on bringing services together to improve access, improve connections between families, professionals, services and providers, and put relationships at the heart of family support.

We can be flexible with start/finish times as long as there is a commitment to a regular working pattern. The position will have a base at our Halifax office (Hanson Lane Enterprise Centre), however, regular travel throughout the borough will be necessary, as a key element of the role is spending time in each of the 14 Family Hubs in 5 areas of the borough.
The Family Hubs Navigator role is a new role and is a time limited pilot project.
This Specialist Service will form part of the Unique Ways offer to the charity’s Parent Carer Members.
We are very grateful to Calderdale Council for funding this new role (part time, term time).
The role is initially funded for 12 – 15 months as a pilot project to test the validity of this new service and to monitor what outcomes can be achieved for Calderdale families that have children/young people with Specialist Education Needs and Disabilities (SEND) via the new Family Hubs network.
